Patients with moderate to severe periodontal disease need a deep cleaning (scaling and root planing) rather than a routine prophylaxis. Doing a routine cleaning on a patient with active periodontal disease does not address the underlying problem and can actually mask symptoms.

Plan for 60 minutes for a returning-patient recall visit, and 60-90 minutes for a new-patient comprehensive exam. Complex cases or first cleanings after a long gap may take longer.
